Purpose:

For any individual who represents a licensed Installer at a job site during the construction of an on-site sewage facility

Description:

This form is for those wishing to become a licensed Installer II. An individual who wants to become an Installer II without the Installer I license must gain experience through the apprenticeship program first and must attend at lest six (6) installations under supervision of a licensed Installer.
